Mentoring Agreement Form

Cancellation Policy for Mentoring Appointments:

Fees for mentoring sessions by Beth Powell, LCSW, are $80 per 60-minute hour, whether the appointments are by telephone or in person.    A deposit for ½ of the estimated cost of services is due at the time the appointment is made to ensure proper cancellation or postponement of services.   This office accepts checks, cash, Visa, MasterCard or Discover Card for in-person appointments.   Visa, MasterCard or Discover Card is accepted for phone mentoring.   Length, cost and topic of the mentoring session will be discussed and agreed upon at the time the appointment is made.   

Mentees are not reminded of their appointments.   Missed appointments (no-shows) are billed in full to the mentee ($80 per hour).    Late cancellations or postponements (changed before appointment time but outside of cancellation grace period below) are billed at ½ of the amount of the scheduled appointment time.   This office is unable to make exceptions for no-shows, late cancellations or late postponements.   To receive deposits back for cancelled or changed appointments, the mentee agrees to give Child-in-Family Services the following notice by phone, not e-mail:

--One-hour appointment:                          1 day notice

--Two-hour appointment:                          2 days notice

--Three-hour appointment:                       3 days notice

--Four- hour appointment:                        4 days notice

--For appointments over 4 hours, estimate 1-day cancellation notice for each hour scheduled.

A $25 dollar fee will be charged for returned checks.   Time charged for the appointment begins at the time the appointment was scheduled.    E-mail cancellations, scheduling, or rescheduling of appointments are not accepted.   One must use the telephone to schedule, reschedule or cancel appointments. The office is closed on weekends and holidays.

General Information:

Please do not come to your appointment more than 10 or 15 minutes early.    For phone appointments you will call the (979) 733-3232 phone number at the appointed time.   The usual hourly rate of $80 is charged to write letters or speak with entities such as judges, attorneys, social workers, probation officers, etc.   The mentor does not go to court.  

What is said in this office will remain private and confidential within the guidelines of HIPAA.   The people who work here are under an oath of confidentiality.   It is a requirement for this office to keep CEU records.   There are exemptions to the confidentiality rule.   In situations involving child abuse, there is a legal and ethical responsibility to report the abuse to Children’s Protective Services (CPS).   If it is felt that someone is of danger to him or herself or others, then an action to protect must happen.    If a contracting agency is paying for services, then it has access to one’s records.
